    What is called Conservative Therapy I call inaction. If your in pain who wants to told to keep their legs elevated when surgical treatment is available, Lol. CVI is like a bad tooth. It doesn’t get better.

      Thank you for your comment. Yes, venous insufficiency is usually quite progressive, however, each patient is unique and conservative therapy may be appropriate in some cases.

      There can be many variables, is the pain from the incision or is it pain in the leg? What procedure did you have? We would suggest if you are having leg pain 2 weeks after the procedure you should call your doctor's office for a follow- up appointment to be evaluated.

    My stent has been misplaced and is protruding across my IVC. And all the problems it creates as the blood clots on it you can imagine. It can't be removed either.👍

      ​​@@mvitv1693 my stenting procedure had been done here in Italy, Milan. My vascular surgeon also didn't insert a guidewire in my right iliac vein to better understand the position of my vena cava so the stent doesn't go far into the vena cava.

      May-Thurner Syndrome is a physical compression of the iliac vein so anti-coagulants will not solve the problem. Stenting is needed to open up the vein and keep it opened.

    What happens after gsv ablation if the vein recanalization happens? Study's on Internet point to that being a long term issue. Will a clot form? It's scary

      The risk of clot formation, known as deep vein thrombosis (DVT), after vein recanalization following GSV ablation is generally low but not entirely ruled out. However, the risk depends on various factors, including the extent of recanalization, underlying venous insufficiency, and individual patient characteristics. Studies have shown that vein recanalization after ablation can occur in a small percentage of cases, and it may require additional treatment to address the recurrent symptoms effectively. The recurrence of symptoms such as pain, swelling, or discomfort in the legs may prompt further evaluation to assess the need for additional interventions. It's important for individuals who have undergone GSV ablation and experience recurrent symptoms to consult their healthcare provider for further assessment and management.

    Good video but something less than complete list of alternative treatments. I corrected arterial plaque with zero intrusive procedures, and zero side effects, naturally . I consumed three enzymes for 3 months using Vitamin K2-MK7, serrapeptase and nattokinase. No scars, no pain. Take a moment to research the above enzymes and draw your own conclusions.     I had blood clots in my leg and when I had a follow-up ultrasound, my clots were gone but they seen something, so I had a ct, and they seen a block artery, I had a more clear ultrasound at a vascular surgeon, seen my blockage and narrowing, it was clear. They said I was down to %60 blood flow to my right leg. My leg changes horrible color's when I stand, and extremely painful. I have a follow-up appointment next week, what should be the next step. I am on two different blood medications, one a blood thinner and another for the block artery. I also had knee surgery in September in the same knee. A stent has been mentioned more than once.

      Treatment is really dependent on what symptoms you are having and your physical exam. This is best determined in person by the provider that you are seeing.

      @mvitv1693 Thank you for responding back. At the moment my leg turns purple,swollen and extremely painful. Yesterday I pushed myself with house work my leg is still in so much pain. I'm also having these cold spots throughout my leg, making it feel wet, really cold. I am on blood medications but it's getting worse. First it was my foot turning purple now it's my whole leg smh. Hard to tell what's what because of the patella reconstruction surgery, blood clots, blocked artery. They all cause pain and swelling. I see my vascular surgeon on Friday. But I came across your video while I'm doing research. Thank you for your video and responding back.

      @mvitv1693 I just wanted to update, I seen my vascular doctor and I have a blockaged in my iliac artery along with narrowing so we will be going in to put a stent in. However it will be going in from both sides so it can remove all the block. I guess I will be staying overnight so they can watch me, because of my high risk of blood clots.       Typically for procedures such as these we use a combination of medications through an IV to keep you comfortable. It is termed conscious sedation, so the goal is for you to be conscious and arousable throughout the entire case, but comfortable.

      Smaller vessels in the lower leg are typically treated with ballooning rather than stenting due to the size of the vessel. Factors such as genetics, diabetes, elevated cholesterol, smoking, and lifestyle can lead to these vessels re-occluding. It is important to walk/exercise daily, quit smoking if applicable, manage blood sugar and cholesterol levels and take medications as prescribed to try to prevent this.       The long term treatment is based on the intervention performed. Balloon venoplasty will not necessarily require any long term medication. Most stenting requires at least short term antiplatelet therapy, but this will be based on your physician's recommendations

    What do you all consider a severe blockage? My moms doctor never gives a percentage just says severe, she’s already had one amputation, bypass, graft and stent, they want to perform an experimental surgery on her other leg and we almost lost her several times and I’m just scared for her. No legs doesn’t leave her with much quality of life after several strokes that has made her weak so moving by arms isn’t an option, she also has been diagnosed with vascular dementia. A lot of this has happened right after we lost my brother and nephew, her son and grandson. It has caused so much stress on me that I had a massive heart attack March 2023. I worry about this everyday! 😢

      We are sorry to hear this and wish you the best. To answer your question, the severity is really determined more from the symptoms and physical exam rather than a percentage of stenosis. A severe stenosis would cause pain both with ambulation and possibly at rest and may be causing discoloration of the skin, wounds or tissue damage. Best Regards.

    I complained to my VA doctor that over the past year that I have increasingly painful legs walking short distances along with SOB. I insisted on seeing a vascular surgeon who never spoke to me and wrote a letter to me that my , at rest claudication test , was normal. Which makes sense to me since I am perfectly comfortable if I were to sit in lazy boy chair all day. I had the double bypass widow maker surgery in 2018. I still think I have PAD. Should I insist on additional tests to secure or rule out PAD? Thanks

      Yes, if you have consistent symptoms of pain in the legs every time you walk, then additional testing is important. The ankle brachial index which is the first test they perform can in some instances have falsely normal results. An ultrasound would be a better indicator in this case.

      Depends on how bad ur veins are. My right leg was easy, my left leg, 25 stitches and it's still aching and uncomfortable after 9 days, no picking up on anything heaver than 5 lbs or strenuous activities for at least 3 weeks . My right leg, already back to normal.

      Yes walking is encouraged following a phlebectomy procedure. This will keep blood circulating and help with healing. If you are sitting for any extended period of time you should elevate your leg.

    Doctor I appreciate your comprehensive video on dvt and leg ulcers! I myself have had dvt for over 40 years now but have been able to manage it with compression. During my early years, I had major problems with open ulcers and they could not be healed until I met a vascular surgeon with a different method. He called it a vascular pump have you heard of it?. It was so simple all he did was cut a piece of foam to fit properly over the ankle and wrap a tensor bandage around the leg and over it to add compression. After years of open ulcers and many different procedures that didnt work the ulcers healed in under 2 weeks. I use this foam to this day. Sometimes the simplest things are the most effective

      Thank you, doctor! It is incredible but most surgeons would hesitate to do any SCLEROTHERAPY with an open ulcer (some would talk about vein stripping or even removing the greater saphenous vein) they would always wait till the ulcer was closed. The only way the ulcer would close is with foam compression therapy. placing firm foam on top of the ulcer and compressing it with a tensor bandage. You too, I wish you the best and urge you to try a little foam :). You may be amazed!@@mvitv1693

    Thank you for this super informative video! I had DVT followed by extreme pain caused by post thrombotic syndrome with reflux in the short saphenous vein and sapheno-popliteal junction. I did laser ablation of my short saphenous vein and now I have more pain, great saphenous vein reflux and varicose veins and so much pain and Im only 24 and compression stockings haven’t been helping. Can I treat all this with laser ablation or other surgical methods when I’ve already done laser ablation of my short saphenous vein? :/ you mentioned theres currently no surgical treatment for deep vein reflux so in my case what could be done for the great saphenous vein reflux in your opinion? Isnt this reflux that causes varicose veins and we should treat the root cause? Thank you very much in advance

      Yes the great saphenous vein and varicose veins can still be treated which could help to lessen your symptoms. With underlying post-thrombotic syndrome and deep vein reflux, you may always have some degree of discomfort, but the goal would be to ease as much of your symptoms as possible. That being said, with the history of DVT and deep and superficial vein involvement at such a young age, I would also question the possibility of iliac vein compression. It might be worthwhile to have further imaging first, usually by CT scan to better evaluate the outflow tract in the pelvis area.

I really hope I find the suitable treatment because several doctors have told me this is a complicated case but this discomfort and achy heavy pain is unbearable and is affecting my quality of life greatly. My DVT was in the external iliac vein down to common femoral vein to superficial femoral vein but my recent ultrasound showed normal compressibility of distal external iliac and femoral veins with patent lumen but there are remnants of the old DVT showing as 'fine fibrous thread within proximal commit femoral veins'..would I still need further imaging? Also after SSV laser ablation shouldn't it not be compressible? but my report says it's patent and compressible when the surgery was supposedly successful? Can I also please know which vein is causing the deep vein reflux or did you mean due to the DVT or could it be reflux in the sapheno femoral junction even tho nothing was mentioned in the report? And usually would there be complications/danger in the future if I don't treat my refluxes and varicose veins regardless of the pain? Thank you so much in advance.

    The vascular surgeon i saw said that there's nothing surgery they can do when it's in the main, large vein in the leg. Do you agree with this or is there something new that can be done now?

      This is a difficult question to answer without knowing exactly what your ultrasound showed. It may be that the main vein that they mentioned is a deep vein rather than a superficial vein. It is correct that there is no surgical treatment for DEEP vein reflux at this time.

      @@jessicacatherine808 I had a catscan and vein ultrasound done and was diagnosed with may thurner syndrome. I have mild to moderate compression on my vein from the artery. No Dvt was seen in my test so my cardiologist said to keep an eye out for swelling/pain in my leg.

    My doctor said my groin pulse in the left leg is weaker than the right. I am now going for an ankle brachial index. Is that test ever normal, yet there is still a circulation issue in the legs?

      The ankle brachial index is a fairly accurate indicator of arterial disease in the legs. If you have a long standing history of diabetes, that can cause calcification of the arteries and you can get a false elevated/normal result. Your doctor will determine based on your results and your symptoms if further testing such as an ultrasound is necessary.

      The procedures to treat venous insufficiency are highly effective at improving symptoms and quality of life. Venous insufficiency is considered a chronic disease process and therefore once you have been fully treated we will continue to follow up with you on a regular basis. The treatment plan and length of time to fully treat the veins varies person to person.

    Why not to use a CT scan to see the diameter of the cava and iliac veins from the rins to the legs?it is not invasive, and doing lots of transversal pictures, you can find out where the compression is.

      CT scan is a diagnostic tool that we do use to diagnose iliac vein compression. We proceed to venogram only if treatment is needed due to the degree of compression and associated symptoms.

    Great job! I am a 65 year old male and have had everything else ruled out for my left leg swelling and reflux. The feet swell more when I am seated at my desk all day, the left a lot more than the right. Being about 20-30 lbs overweight is it possible that compression could be greater in a seated position and if so, can a test be done to confirm this in the seated position? Thank you in advance.       With leg pain, it is a good idea to visit a vascular surgeon who is board certified in vascular surgery to have them check your circulation. The sooner, the better, because vascular problems can be progressive.

    On Wikipedia it says despite numerous treatments. Treatment for cvi remains unsatisfactory. So what is the end goal of treatment? Is it just temporary. Nobody can seem to answer the hard questions. My surgeon moved out of state. Left me without a answer. Is it true venous insufficiency is the most difficult thing to treat?

      Depending on the extent of your venous disease, sometime multiple treatments are needed to successfully treat your problem. The end goal is helping you to feel better and reducing your leg symptoms. It is not a temporary fix, however when you have a history of venous disease you are prone to develop new veins down the road. Venous disease is a chronic disease that requires ongoing management. Once your treatments are complete, we recommend routine follow-up visits where we can catch issues early as they are developing so any future treatment won't be as involved.

    What would cause a vein to reopen after evlt or rfa? I've read alot of people say there vein reopened months later or even a couple years and they now have swelling or pain they never had before then what happens??

      Yes this can occur months or even years after an initial procedure, though it is not the norm. When this happens it is often due to a perforator vein in the area. A perforator vein is a connecting vein between the superficial and the deep system. If the perforator vein becomes incompetent it can put a lot of pressure on the closed superficial vein and over time the pressure can cause the vein to recannulize. More often than not however if someone is having retreatment down the road it is not on the same vein that was previously treated. Often it is a new superficial or varicose vein that has developed. When you have a history of venous disease, you are more prone to developing varicose veins. This is why we recommend routine follow-up once your vein treatments have been completed. This way we can catch something early as it is developing and treat it before it becomes a bigger issue.

    I must be the unluckiest loser on the planet. Last year I had to have my right big toe amputated due to gangrene that also brought osteomyelitis. I also had 2 stents placed on my right leg by 2 different surgeons. I was told it would improve my quality of life and help me walk better;also it would help heal the amputation. When 2 months had passed and the wound would not close,even after the use of wound vacs;it was decided for me to have 2 more stents placed....2 on each leg....It didn't help me walk better and stilll took over 16 weeks for the surgical wound to heal. I am now experiencing even more troubles walking and being able to stand without toppling over due to balance issues that my newly-deformed foot worsened. I am now looking at a permanent wheelchair situation;and an early death because I can't even do my exercise bike anymore due to increased pain in the legs;therefore my blod pr esure and glucose /a1c levels will hit the stratosphere. If there's really a God...I hope for mercy and allow me a sudden and quick and effective widowmaker stroke.
