Market Medicine? How Money is Changing the Experience of Medicine and What We Can Do About It
Some observers have argued that present day America is not just a society making use of a market #economy, but in fact a “market society” where all individuals and institutions are forced into cutthroat competition for #profits and survival. This situation, if real, will inevitably shape #healthcare organizations, and within them, interpersonal interactions like those between #patients and their #caregivers. These interactions might be forced into transactional, impersonal modes that lead to widespread experiences of moral injury, burnout, loneliness and distrust. And indeed, these experiences seem to be on the rise amongst medical practitioners. Some have talked about the moral crisis of American doctors. This panel examined the crisis through theoretical, clinical and ethical lenses, asking how it is manifesting, what is causing it, and what we might do about it.
Discussants included Ian Corbin, PhD, MA, Lydia Dugdale, MD, MAR, Jonathan Cahill, PhD, MA, and Omar B. Lateef, DO. Consortium organized by Kelsey Berry, PhD. Support provided by the Oswald DeN. Cammann Fund at Harvard University.

      I wonder this also. Don't get me wrong, it's tragically obvious that in most cases it's the "facilitator" doing the writing. However, I'm wondering if there are valid cases of some people who are physically impaired but otherwise high-functioning. For instance, sometimes there are accident victims who learn to communicate using a screen and eye movements or breathing straws. It's a shame this sham aspect might discredit rare circumstances in which it might actually be applicable.

      I think the first case of it and any case in which the facilitation is genuinely just a hand underneath an elbow, that this works, those seem compelling enough and I wish those cases would also be tested. But any case in which the hand or forearm is being supported, whether the person being facilitated is mentally competent or not, the actual typing is always influenced mostly by the facilitator.

    If any person is interested in evil and horrors, just research the history of psychiatry. You'd think that doctors would have a history of ethics, and in some cases they do... but at the same time, some of the greatest and most disgustingly evil atrocities and human Rights violations have been committed in the name of psychology/medicine... and I think the worst of the worst have been in psychology specifically.

    I know a man who had a snowmobile accident and was in a permanent vegitative state for 3 years, tube fed in a hospital bed at his home. His wife took care of him. 3 years later he just woke up, started talking, sat up learned to walk. He was slow but he was aware the rest of his life.

    5 years later,Medicare is going broke.Insurance companies are crooked as heck. Older people are being cheated like crazy. I detest all. We put money into Medicare all the years we worked...it is not an entitlement.When I first started collecting Medicare ,it was normal.They paid first and what they didn't pay,my secondary insurance took charge of.NOW my insurance took over Medicare and no place even takes my Medicare card!! That really sounds odd to me. Like I said...I do not trust insurance companies at all...I have been burned by them on occassion.

    There is no ethics when it comes to Harvard University, solar, radiation solar modification, solar radiation management, chill engineering, space bubbles, whatever you wanna call this grotesque experiment. You walk outside in Massachusetts and it is a blanket of sickness of chemicals that when you breathe it’s so thick and irritating your sinuses, your throat, and your lungs are instantly inflamed. When you exhale, there’s an aftertaste of chemicals. This is not ethical, nobody consented to this mass experiment.

    Y’all are thinking about this all wrong! Think of frequencies & Vibrations. Not this modern square stuff. Think of old World buildings, lots of beautiful arches, domes etc. like old cathedrals and the Capitol Buildings. That’s what will make people healthy and happy. Of course, our health system is a whole other problem, it’s bad for one’s health period!!

    RIP. Amazing, courageous man. As a nurse, being forced to keep some seniors alive with severe dementia makes me sad. These people have zero quality of life. Can't walk, talk, or even eat without a feeding tube. Usually have a bunch of painful bed sores, too. But most of these unfortunate people have family members demanding to keep them alive. For what? To stare at 4 walls? Just because we can keep people alive, doesn't mean we should.

    this is why I won't ever be an organ donor. once you put that on there they dont even try. too much profit made from fresh organs vs keeping one alive (profit loss). I would do it if my family got paid what they would charge for the procedure/organ. other than that no thanks. Canada goes 5 minutes after "declared" death that's crazy - seem like a organ harvesting scheme.

      I do she lived on after they removed the oxegen support. She was always tube fed, never regained consciousness, was in a fetal position.

    A Californian teen, Jahi McMath made headlines after she underwent a surgery to treat her obstructive sleep apnea which is a condition where your breathing is interrupted while sleeping. After surgery to remove her tonsils, adenoids and sinus tissue she soon became unresponsive after suffering cardiac arrest in the ICU. Just a few days later, Jahi was pronounced brain-dead due to a tragic brain hemorrhage and anoxic brain injury. Her parents, mistrusting of the medical system, refused to believe this statement. Many months after declaring Jahi brain-dead, challenges and arguments ensued. The hospital and physicians believed it was reasonable to remove Jahi’s ventilator and feeding tubes due to her being brain-dead, however, her family refused that her ventilator be removed stating that Jahi showed signs of life. Many battles were faced between Jahi’s family and the court. Who gets to decide to remove the ventilator? The court? The hospital and physicians? Jahi’s family? Should families or surrogates, rather than physicians and the court decide when patients are dead? Jahi was soon transferred to a different hospital for continuity of care before later dying at home surrounded by family. So what is the definition of brain death? A patient is considered brain dead if the individual has irreversible cessation of circulatory and respiratory functions or all functions of the central nervous system including the brain and brainstem. Even though Jahi was considered brain-dead, she still responded to external stimuli. For example, when ice was placed on her foot, she moved her leg away from the cold stimulus. I believe the correct term for Jahi is a persistent vegetative state. A vegetative state means that an individual is awake but not showing signs of awareness due to brain damage. I think an important consideration to think about in this situation is what the patient would have wanted ultimately in the end given the circumstances. Was the mother wanting to keep Jahi alive artificially, confined to a bed and unable to interact or speak for her own benefit? Another ethical issue that needs to be considered is the fact that Jahi was 15 and considered a non-responsive minor, lacking autonomous decision-making. Therefore, her mother is the ultimate decision-maker, making the decision for the best interest of Jahi. In ethics, I believe it is so important to always consider the patient’s viewpoints and what they might want in a given situation. If they had autonomy, what decision would they make? As future physicians, it is crucial to always do what is best for the patient and have their best interest in mind.
