Central East CCAC (Community Care Access Centre)
What we do:
Our goal is to connect people with the right care at the right time and in the right place.
Every day, we help people find their way through Ontario's health care system, understand their options and connect them to quality community-based health care and resources.
Our highly qualified health professionals provide a range of specialized care at home and in the community, including physiotherapy, nursing, speech therapy, palliative care and more. We work with people of all ages to ensure they make informed choices about their care and receive services in the most appropriate setting.

Loved this team!
my son is 11yrs with charge and i am happy his health got better since he was born. He attended CSDR at riverside. That school has done marvelous things for him. I must tell you charge kids demand alot of attention because their health is fragile especially when their born.
I have CHARGE syndrome
But that other little guy sitting at the back. He doesn't look too happy or included. Says a lot
I wish we would have this process in the U.S soon. I wonder if the CDI uses the shared appointments method as well. Good luck. It looks very promising.
