Based on how his quirk has been described I assume that if he has to regenerate while starving to death his body straight up breaks reality. His quirk requires energy so I assume that in a scenario such as that his body would self-canibalize at an uneven rate of exchange. Basically, he’d regenerate more of his cells than his body consumes for energy which would result in a slower healing process that’s basically defying the laws of physics. It’s basically creating energy to allow for regeneration since simply transforming his cells into calories and using those same calories to regenerate cells would result in an endless loop. Also, since he didn’t have the joint and his quirk appeared before “quirks” appeared maybe what he has isn’t a quirk but a beta version of a quirk. Maybe it’s a dormant mutation that nobody else awakened, but evolved into quirks which did actually awaken. Either way his quirk would still kinda nerf all for one since it would keep him in his injured state and probably prevent him from collecting more quirks pre editing them. I might be reading into it too much and it might just be unable to be stolen since his each of his cells is conscious.

i dont know it may just be my interpretation but it feels like OFA is representing life/good, AFO represents death/bad and izuku represent the in between of the other two as he saves people(good) but also kills to save which is considered bad, again just my interpretation of this but still.
