I beg to differ on this statement. There is nothing called blind Correct your terminology It is called “ anatomical landmark guided Technique “. You need anatomical knowledge to perform the procedure. And there is a difference between a technician and anaesthesiologist. “Koi bhi” kar lega bina knowledge k. Complications bhi karega. The video of landmark guided technique is there on my channel. Kindly search it.
Hi..how to preserve the motor function of the leg if we do adductor canal block? Do we block more distally?
Hi. Thank you for posting your query. True Adductor Canal Block (ACB) doesn’t cause any motor weakness. We are blocking only saphenous nerve proximally, and popliteal plexus block via distal spread. we use low concentration like 0.2% Ropivacaine with adjuvant dexamethasone. Nerve to Vastus Medialis (NVM) travels outside the true AC. Even if you block with low concentration LA, no issues. Again there are multiple branches supplying the VM muscle. we target NVM in distal femoral triangle block, where it is separated from Saphenous nerve by a thin fibrous septa.

Can u say the direction of needle at the time of insertion and how to progress the needle once inserted??
Hi Dr. Sourav I have mentioned in the video about the needling technique. From lateral to medial. You can advance the needle through non-neural tissue, place it near neural elements and deposit the LA. Alternatively you can use hydrodissection while advancing the needle towards neural elements.

Sir as per recent change in nomenclature of blocks by Delphi method, proximal sciatic blocks are named as Sacral plexus block, Transgluteal, Infragluteal and Anterior approaches. Where does GT-IT approach falls as per this new nomenclature?
Transgluteal approach as we are performing the injection deep to the Gluteus maximus muscle
The name subgluteal nerve block was harmonized with the transgluteal approach. The rationale behind choosing the transgluteal approach was to avoid confusion between subgluteal and infragluteal. Transgluteal infers the needle going through the gluteal muscle, while the infragluteal infers needle insertion with a trajectory toward to the sciatic nerve target inferior to the lower border of the gluteal muscles.
Only your video is required for revision of these proximal approaches These difficult proximal approaches looked cake walk to us in those 16mins

is superficial sufficient for clavicle fracture analgesia
Clavicle is supplied by the following nerves: Osteotome: 1. Supraclavicular (C3-C4) 2. Lateral pectoral (C5-C7) 3. Spinal Accessory (CNXI) 4. Nerve to subclavius (C5-C6) 5. Axillary (C5-C6) 6. Suprascapular (C5-C6) Myotomes: 1. Axillary nerve (C5-C6) 2. Suprascapular nerve (C5-C6) 3. Lateral Pectoral nerve (C5-C7) 4. Nerve to subclavius (C5-C6) 5. Spinal accessory nerve (CN XI) 6. Trapezius branch of cervical nerves (C3-C4) Dermatome: 1. Supraclavicular 2. Ventral Rami of C5 Superficial Cervical Plexus only covers Supraclavicular nerve.
What is about intermediate CPB, regarding area of coverage ?. @@DrTuhinM

For SIFI, if you want block obturador nerve, you should use 40 mL of dilute anesthetic. PENG you could use 20 mL of dilute anesthetic, for LFCN 05 mL. I use diluted anesthetic because they aren’t planned for anesthesia, just analgesia.

Correction in the Video: In Subgluteal Space Contents, read ACFA as Ascending circumflex femoral artery( a branch from medial femoral circumflex artery which is a branch of profunda femoris artery). I have mentioned is as anterior circumflex femoral artery, which is wrong. I apologize for this unintentional mistake. 🙏🏻
